Vulvar Cancer

Cancer of the external female genitalia, most often squamous cell carcinoma. It arises by two distinct routes: an HPV-driven pathway affecting younger women, and a pathway from chronic lichen sclerosus and differentiated VIN in older women, which carries a worse prognosis. Diagnosis is frequently delayed because symptoms are attributed to thrush or menopausal change.

Certain rare malignancies progress rapidly or carry acute risk of airway obstruction, acute spinal compression, hydrocephalus, or biopsy-seeding. Do not perform needle biopsy or surgery outside of an NCI-designated specialty sarcoma or neuro-oncology unit without multidisciplinary tumor board review.

Report immediately to an emergency department or tertiary oncologist upon:

  • Vulval itching lasting beyond a few weeks in a postmenopausal woman warrants examination and biopsy, not repeat antifungals

Prognosis & Disease Trajectory

Objective clinical outlook without false reassurance

Good when caught early. Five-year survival exceeds 85-90% for node-negative disease but falls to roughly 50% with inguinal node involvement and lower with multiple involved nodes. The dominant problem is presentation delay, not treatment failure.
