Liver Cancer (HCC) Treatment in India & US: Symptoms, Stages, Options, Cost

Hepatocellular carcinoma (HCC) is the most common type of liver cancer. Treatment depends on tumor size, location, liver function, and overall health. ByOnco helps you navigate treatment options, find the best hospitals, and access expert second opinions.

Symptoms & Early Warning Signs

Common indicators and clinical warning signs

Unexplained weight loss
Loss of appetite
Upper abdominal pain or discomfort
Nausea and vomiting
General weakness and fatigue
Jaundice (yellowing of skin and eyes)
Swelling in abdomen (ascites)
Itchy skin
Dark urine

Note: Early-stage liver cancer often has no symptoms. Symptoms usually appear when the cancer is advanced or when there's underlying liver disease. Regular monitoring is important for high-risk individuals (cirrhosis, hepatitis B/C).

Diagnosis Pathway

Step-by-step clinical evaluation and staging workflow

1

Medical history and physical examination

2

Blood tests (liver function tests, AFP tumor marker)

3

Imaging tests (ultrasound, CT scan, or MRI of liver)

4

Liver biopsy (if needed for confirmation)

5

Staging assessment (BCLC staging system)

6

Assessment of liver function (Child-Pugh score)

7

Evaluation for underlying liver disease (hepatitis, cirrhosis)

Staging Explained

How tumor size, lymph node involvement, and metastasis are graded

Stage 0-A

Early stage: single tumor or up to 3 tumors <3cm, good liver function, good performance status

Stage B

Intermediate: multiple tumors or larger tumors, good liver function, no symptoms

Stage C

Advanced: tumor has spread to blood vessels, lymph nodes, or distant sites, or patient has symptoms

Stage D

End-stage: very poor liver function or very poor overall health

Treatment Options

Standard of care modalities according to NCCN clinical guidelines

Surgery (Resection)

Removal of part of the liver containing the tumor. Only possible if tumor is small, confined to one area, and liver function is good. Requires healthy liver tissue remaining.

Liver Transplant

Replacement of entire liver. Best option for early-stage HCC in patients with cirrhosis. Requires meeting specific criteria (Milan criteria or similar) and being on transplant list.

Ablation

Destroying tumors without surgery. Methods: radiofrequency ablation (RFA), microwave ablation, or ethanol injection. Used for small tumors (<3-5cm) that can't be surgically removed.

TACE (Transarterial Chemoembolization)

Procedure that delivers chemotherapy directly to the tumor and blocks its blood supply. Used for intermediate-stage HCC. May be repeated multiple times.

Targeted Therapy

Oral medications: sorafenib, lenvatinib (first-line), regorafenib, cabozantinib (second-line). Used for advanced HCC. Can slow tumor growth and extend survival.

Immunotherapy

Atezolizumab + bevacizumab is first-line for advanced HCC. Other options: nivolumab, pembrolizumab. Used for advanced disease that can't be treated with surgery or local therapies.
